Arrangements announced for Michelle Eustis

Columbus, OH — Michelle Eustis, 25, of Crete, Ill., died April 11, 2011, at Stroger Hospital in Chicago.

She was born Oct. 27, 1985, in Harvey, Ill., the daughter of Cynthia and Charles Eustis, and was an avid horse enthusiast and had a great love for all animals.

She is survived by her parents; daughter, Haylee Koifoid; brother, Cody; sisters, Taylor and Jaquelyn; niece, Payton Bernhal; half brother, Charles IV; and half sister, Tracey (David) Frost.

Visitation will be held Monday, April 18, from 9 a.m.–2 p.m. at Hack Funeral Home. Funeral services will be Monday, April 18, at 2 p.m. Burial will be in Skyline Memorial Park.

Memorial contributions may be directed to her daughter for an educational fund which has been established at First Community Bank and Trust (c/o Michelle Eustis-Haylee Koifoid), 1111 Dixie Highway, P.O. Box 457, Beecher, IL 60401.
