Dayton, OH – Attention horse persons racing or who have raced at Hollywood Gaming Dayton Raceway.  The end of the meet is fast approaching with only 4-1/2 weeks remaining.  In order to get everyone paid, we need a completed purse request, a W-9 and a voided check for the account receiving the funds.

If you are located out of the country, a paper check will be mailed to you after you have completed a W-9.  Completed forms may be returned via fax 937-235-7895 or emailed to terri.mtpleasant@pngaming.com

The deadline for ACH payments or printed checks is Jan. 7, 2021.
