Dayton, OH — It’s the last call for completed purse request and W-9s for the 2015 fall meet at Hollywood Gaming Dayton Raceway.

You may print the forms from the Hollywood Gaming Dayton Raceway website.

Wednesday (Jan. 13) will be the last day checks will be printed.

If your completed forms are not received prior to this date, you will have to wait until racing resumes next September.

You can either fax your forms to 937.235.7895 or scan and e-mail to the horsemens’ bookkeeper at Terri.mtpleasant@pngaming.com.
